If you are a Florida Blue Preferred HMO member: Call 1-844-783-5189 (TTY 1-800-955-8770). We are open from 8:00 a.m. – 8:00 p.m. local time seven days a week.
Caregivers: When a request for a grievance or appeal is submitted by someone other than a member, an appointment of representative form form or other type of legal representative document may be required.
